Subcontractors

Cornerstone works with some of the best subcontractors, disciplinary specialists, and vendors in the business. If you’re new to our market and would like to be added to our bidders list, please fill out and submit our subcontractors & suppliers pre-qualification questionnaire.

Please have the following supporting documentation ready:

• Signed W9 (available at www.IRS.gov) is requested, but not required;
• OSHA forms 300A and 300 (omit employee names) for the past three years is requested, but not required;
• EMR verification from your worker’s compensation provider for the past three years is requested, but not required;
• Safety Manual (reference only);
• Professional license certificates (general contractor, engineer, etc.), if applicable; and
• Financial statements for your most recent fiscal year-end are requested, but not required.

NOTE: uploading blank/empty files or the same file for the W9, OSHA forms 300A and 300, and EMR letter will create an error in the system and you will have to re-enter ALL information again.

You have the option to save your progress or to completely submit your pre-qualification. If you choose to save your progress, you MUST copy/save the hyperlink that’s provided on screen or you will have to re-enter all information again.
